Initial evaluation/ consultation/ treatment:

Follow up visits:

60 min :

$140.00

Insurance:

We do not accept insurance any longer, however we will be

happy

to provide you with recepts including codes to submit

to your

insurance company.

Cancellation Policy

If you need to reschedule please be sure to call the clinic 24

hours

before your scheduled appointment.

If you do not cancel or reschedule at least 24 hours in

advance- or

if you miss your scheduled appointment you will

be charged a

cancellation fee of $80.00.

We accept all major credit cards, cash and personal

checks.
