If you need to reschedule your appointment please be sure to call the Dr. Office 24 hours before your scheduled appointment time. If you do not cancel or reschedule your appointment at least 24 hours in advance - or if you miss your scheduled appointment you will be charged a cancellation fee of $160.00

We no longer accept insurance, however we will be happy to provide you with receipts including codes to submit to your insurance company.
